Niche Positioning

The Therapist Niching Framework: Score, Test, and Validate Before You Commit

The evaluation methodology in detail — because choosing a niche should feel like informed clinical decision-making, not a marketing gamble

The biggest mistake therapists make with niching is treating it as a one-time decision instead of a testable hypothesis. This framework gives you the methodology to evaluate, test, and validate a niche before investing thousands in training or rebuilding your entire online presence.

The Scoring Rubric

Rate every niche you are considering on three dimensions, each scored 1-10:

3-Circle Scoring Rubric
Dimension1-3 (Low)4-6 (Moderate)7-10 (High)
EnergyThese clients drain you. Sessions feel like obligation. Some sessions are good, some are neutral. No strong pull. You look forward to these sessions. The work energizes you.
InterestYou have no desire to study this further. CE feels like a chore. You are mildly curious but not driven to learn more. You read about this outside of work. You attend conferences because you WANT to.
DemandNobody is searching for this. No referral pipeline. Some search volume. Occasional referrals. Market exists but is small.People are actively looking. Directories show demand. Colleagues ask if you take referrals.

Interpreting Your Scores

Score Interpretation Guide
PatternWhat It MeansAction
All three 7+Strong alignment. This is your niche.Name it. Own it. Start the soft pivot.
One dimension below 5Partial alignment. One area needs work. Can you RAISE the low score? If Energy is low, this is not your niche. If Interest is low, invest in training. If Demand is low, narrow the geography or adjust positioning.
Two dimensions below 5Wrong territory.Do not invest here. Return to the caseload audit.
High Demand, low EnergyBurnout trajectory. This is the most dangerous pattern. Market demand without personal alignment leads to a full caseload you resent.
High Energy, low DemandHobby niche. You may love this work, but if nobody is searching for it, your marketing has no fuel. Consider whether adjacent positioning could create demand.

The Caseload Audit

Before you can score anything, you need data. The caseload audit is a structured observation process that generates the information you need to make informed niche decisions.

If you are newly licensed: You do not have enough caseload data yet. That is fine. See a range of clients for 6 months while running this audit. The patterns will emerge. Do not force a niche before the data exists.

The 90-Day Soft Pivot Protocol

Once you have a hypothesis (a niche scored 7+ on all three circles), test it. The Soft Pivot changes your marketing FIRST and your clinical work SECOND. Nothing burns. Nothing blows up.

90-Day Soft Pivot Timeline
WeekActionTime Investment
Week 1Rewrite Psychology Today profile toward the test niche. Change language, not listed specialties.1 hour
Week 2Update website copy: homepage headline, about page, and primary service page.2-3 hours
Week 2Adjust your referral script to use experience-based language instead of specialty lists.5 minutes
Weeks 3-12Continue seeing all current clients. Track every new inquiry: who called, why, and how they described what they needed.5 min/week
Week 12Evaluate: Are new inquiries more aligned? Are sessions better? Are referrals coming from the right sources?1 hour

Before (specialty list): “I take adults with anxiety and depression.”

After (experience statement): “Send me anyone who is performing competence while privately falling apart — the ‘I am fine’ crowd.”

The second version makes colleagues think of a specific face. The first version triggers nothing. Specificity creates referrals.

Live Example: Your niche doesn't always have to be a clinical population. It can be a philosophical stance. For example, Enlitens uses their Radical Transparency & Open Source Philosophy as a filtering niche. It aggressively repels clients looking for the traditional medical model, and magnetically attracts clients looking for collaborative partnership.

Validation Criteria: How You Know It Worked

Post-Soft-Pivot Validation Checklist
0/6

You do not need all six. If four or more are true after 90 days, you have found alignment. Deepen the niche: invest in specialized training, create dedicated service pages, join relevant professional communities.

When to Iterate vs. When to Commit

4+ validation criteria met after 90 daysCommit. Invest in training. Build dedicated pages. Go deep.
2-3 validation criteria metIterate. Adjust the language, narrow the population, or shift the positioning. Run another 90-day cycle.
0-1 validation criteria metPivot. This is not your niche. Return to the caseload audit and test a different hypothesis.
Great validation metrics but Energy score is droppingPause. Revisit the 3-circle scores. Market alignment without personal alignment is a burnout trajectory.
Energy and Interest are high but inquiries are not comingAdjust positioning, not the niche itself. The match may be right but the language may be wrong.

Remember: Most therapists need 2-3 cycles to find deep alignment. That is 6-9 months of testing. It feels slow. It is infinitely faster than spending years as an undifferentiated generalist or investing thousands of dollars in training for a niche that drains you.

Frequently Asked Questions

Score it on three dimensions: Energy (do these clients energize you?), Interest (are you genuinely excited to study this topic deeply?), and Demand (are people actively searching for this help?). Each rated 1-10. If all three are 7+, you have strong alignment. Then run a 90-day soft pivot test: change your marketing language, track who calls, and evaluate the results before committing to expensive training.
A meaningful soft pivot test takes 90 days. In that time, you adjust your marketing language (not your clinical work), track new inquiries, evaluate session quality, and measure referral alignment. After 90 days, you have enough data to either deepen the niche or adjust and run another cycle. Most therapists can run 2-3 tests per year.
